CLAIMS

Claim forms must be submitted using the following form, where it will be automatically submitted to our claims department.

In addition to this form you MUST submit photos and the original receipt of purchase to the email address :

Claims@midtowndezign.com

Claim Procedure:

  1. The customer fill out attached claim form completely

  2. The customer must submit photos, original receipt of purchase and ALL that is requested is on our claim form. 

  3. Claims will not be processed until we have ALL the information so we can review the complete claim and make a determination of how to proceed.

Additional Important Information:

  1. All Claims should be submitted within 30 days of customer complaint

  2. All claims must be submitted to the Midtown DeZign District Corporate office to be considered

  3. All claims must be submitted with sample of defective products

  4. Midtown DeZign District reserves the right to inspect all claims

  5. Midtown DeZign District may require copies of invoices

  6. Claims approved for payment will be paid by credit memo only
